Shannon Smith, Provider Relations Background: Shannon has been working in the healthcare industry since 1991. She has over 15 years of experience working in skilled nursing facilities in both Admissions and Social Work. For the past 5 years, she has been practicing as a Case Manager working with seniors and their loved ones and assisting them to navigate through difficult healthcare systems and supporting them through making the sometimes difficult life decisions they are faced with. Shannon has the wonderful ability of developing trusting relationships with seniors and their families. She is a concerned individual who quickly assesses for needs and helps seniors find available community resources to meet those needs. Shannon has many years of hands-on experience working with patients who have dementia, their families and caregivers. She is very empathetic to their unique needs. Her experience and vast knowledge of senior resources makes her a valuable asset to seniors desiring an expert to collaborate with in order to attain a high quality of life while maintaining the most independence possible.
